Preparing a LEV Thorough Examination and Test (TExT) Report or Commissioning Report for BOHS qualification submission can be one of the most challenging aspects of the P601 and P604 assessment process.
Candidates are required not only to demonstrate that measurements have been taken correctly, but also that they understand how to evaluate system performance, interpret results, assess adequacy of control and communicate their findings in a structured and professional manner. A successful report should provide clear evidence of technical competence whilst demonstrating compliance with the principles contained within COSHH Regulation 9 and HSG258.
This report template has been developed from many years of supporting LEV practitioners through the P601 and P604 qualification process. It provides a structured framework for recording observations, documenting measurements and presenting conclusions in a format that reflects recognised industry practice.
The template should not be viewed as a substitute for competence or technical judgement. Every LEV system is different and reports should always be adapted to reflect the specific system, process, hazardous substances and workplace conditions being assessed. Candidates should ensure that all findings, conclusions and recommendations are supported by appropriate evidence.
For practising LEV engineers, the template may also provide a useful starting point when developing internal report formats or reviewing existing documentation. Clear, consistent and defensible reporting remains one of the most important outputs of both LEV commissioning and statutory Thorough Examination and Testing activities.
Whether preparing a BOHS qualification submission or undertaking workplace examinations, the objective remains the same: to produce a report that demonstrates how the LEV system was assessed, what evidence was obtained and whether adequate control of exposure is being achieved.
